My epoxy terrazzo tiles are not stick to my concrete subfloor anymore. I think that moisture is coming through the concrete and it caused the adhesive to loosen. What can I do about it?

What type of adhesive was used? It sounds like mastic. I would use an epoxy or polymer-modified thinset, which won't dissolve in water, to restick.

It was mastic.  I just had my floor installed about 3 months ago.  Should my installer be responsible?

If an organic mastic was used any moisture coming through the subfloor could certainly loosen or prevent the mastic from curing. I would have never used mastic, only epoxy if the tiles are epoxy based or polymer-modified thinset if they are cement based.
